Removal of a Large Thoracolumbar Epidural Arachnoid Cyst and Recapping Laminoplasty Using Miniplates

Abstract
Spinal extradural arachnoid cysts are usually asymptomatic and rarely expanding lesions. En bloc excision of the cyst, including the cyst wall and complete repair of the dural defect, is the treatment of choice for symptomatic spinal extradural arachnoid cysts. The authors report a case of a 32-year-old man who presented with back pain and intermittent clumsiness in both lower extremities. Radiologic studies revealed a huge extradural arachnoid cyst extending from the T12 to L2 level. Total excision of the arachnoid cyst and complete repair of the dural defect were achieved, after en bloc excision of the spino-lamina complex at the T12-L2 level. The excised spino-lamina complex was recapped at the primary site and fixed using miniplates. Recapping laminoplasty facilitates complete exposure of huge extradural lesions and enables reconstruction similar to normal anatomical elements with less violation of the posterior elements and providing coverage of the neural elements.
